| The following information regarding Mental Health and Substance Abuse Care applies to all plan options that have the managed mental health and substance abuse care benefit which is administered by Cigna Behavioral Health.
With this plan, members have a choice of using a Provider for services within the managed behavioral care network or a Provider outside of the network. If the managed care network is selected, benefits are paid at 100% for Medically Necessary services and there are no predetermined limits (except for the General Lifetime Maximum). If a Provider outside the network is utilized, benefits are subject to the out-of-network deductibles and coinsurance levels. In addition, there are limits on the number of visits allowed each plan year.
Covered mental health and substance abuse care services include, but are not limited to, psychological or psychiatric treatment of the following conditions:
- alcohol and substance dependence and/or abuse
- anxiety disorders/stress
- adjustment disorders
- depression and manic depressive conditions
- eating disorders
- family and marital problems (in-network only)
