| Dental benefits are automatically provided at no cost to employees after six months of active service. At that time, an identification card and booklet are sent out to each employee. The dental plan is administered and claims are paid by Northeast Delta Dental which is located in Concord, New Hampshire.
The Dental Assistance Plan is designed to pay 100% of preventative services with no deductible. Restorative services, bridges and crowns are covered under a Schedule of Benefits, after an annual deductible is met. The deductible is $25 per person, with a maximum of $75 per family. Orthodontia services are paid at 50%, with a lifetime maximum of $1,750 per individual. Maximum benefits for all services other than orthodontia are $1,000 per person, per year. The Dental Assistance Program plan year runs from 7/1 to 6/30 of each year, consistent with the State's fiscal year.
Covered dental expenses are costs incurred by you and/or your eligible dependents for charges made by a dentist for any dental service provided by the plan, if the dental service:
- Is performed by or under the direction of a dentist; and
- Is essential for the necessary care of the teeth; and
- Begins while you or your eligible dependents are covered under the plan.
The term "dentist" means an individual who is duly licensed to practice dentistry in the state where the dental service is performed and who is operating within the scope of his or her license.
