Life Insurance
 ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE

Life Insurance and Accidental Death and Dismemberment Insurance is available to employees at very low group rates. These programs are administered and claims paid by Minnesota Mutual Life Insurance Company.

Life insurance coverage is two times your annual base salary, rounded down to the nearest $100. For example: Annual salary = $25,319 x 2 = $50,638. The Life Insurance benefit would be $50,600. The Accidental Death and Dismemberment (AD&D) benefit is an additional two times your annual salary. AD&D pays benefits in addition to Life Insurance if you die as the result of an accidental injury.

Employees contribute 25% of the premium. The State pays the other 75%.

Employees need to sign up within the first sixty days of employment. If done at that time, coverage will be automatic. If you enroll after the first 60 days, you will be required to provide evidence of good health before coverage can be approved.
