Helpful Hints for Vision Care Visits
When you visit your eye care provider, be sure that a procedure code is written on your claim form.

New Patient Procedure Codes:

92002 Intermediate Exam
92004 Comprehensive Exam

Established Patient Procedure Codes:

92012 Intermediate Exam
92014 Comprehensive Exam

When you are ready to purchase frames* and lenses, be sure that your bill itemizes the charges.

For Example:          Eye Glass Optician Company

Frames                       $150.00
Lenses                        $200.00

Total                           $350.00
Tax                              $ 17.50

Grand Total                $367.50

* As a reminder, frames are not covered under your benefit plan.
**Be sure your provider includes names and tax id number on the claim form.
